including Rehabilitation of Offenders Act declaration
CV     
Covering employment and gaps since age 16 and details verified (see Safeguarding)
Proof of ID       
Passport (both cover and Photo Section) Driving Licence or Birth Certificate
Proof of Address  
Driving Licence: Utility Bill or Bank Statement issued within last 3 months
Right to Work     
UK Passport or Other Right to Work (see Right To Work List)
Proof of NI Number
NI Card, P45/P60 Letter from Job Centre
References    
5 Years minimum (Including two professional )
CRB   
Issued to DASH within last 12 months
Police Check**
Non-EU Candidates and those who have lived abroad (see “Overseas Police Checks)
